Working with Kaiser Permanente, Hunger Free Colorado helped create a system for medical providers to screen patients for food insecurity. If a patient screens “positive,” the medical provider sends a referral to our statewide food resource hotline.
Our food assistance navigators then proactively calls each patient, connecting them to food resources in their community, screening for federal nutrition programs, and helping them apply for food stamps, if they qualify. By actively contacting patients instead of passively waiting for phone calls has resulted in an increase of successful connections from 5% to 78%!
Hunger Free Colorado currently partners with more than 25 different organizations for the hotline referral program, and more than 50% of all calls are medical referrals.
